*NEW* We Now Use Paperless Forms!
In an effort to cut down on paper use in the office, our patients now use a convenient computer kiosk in our office to fill out paper work. Our computer system makes filling out the forms as easy as the paper forms. However, if you are not comfortable or are unable to use the computer, we are available to help.
Please bring the following to your first appointment:
1) Dental Insurance Card or name of the insurance and your identification number
2) A list of ALL medications that you are on including vitamins and non-prescription medications.
3) The name and phone number of your current physician.
4) If possible, bring copies of your x-rays with you. We prefer to have any x-rays e-mailed to us at aimee@implantsbydrfletcher.com.
